Claim your prizes for participation at banner_contest here. The prizes will be handed out once the moderators for the community have confirmed that the person in question has contributed.
Cards earned through submitting, voting and winning will be handed out as soon as results for that week come up, and details of the transactions can be included in the activity logs.
Prizes are:
Submitting - Submitting one banner will earn you two random cards. Two banners earn you three random cards. Three and above banners earn you four random cards.
Voting - Voting (critiques are necessary) will earn you two random cards.
Winning - First place winners will win two random cards. There is no second place or under, so if there are ties, all winners will get the prize.

Community: The name of the community you contributed in - just to make sure you're in the right place.
Contribution: What you did. Put a text link (do not use HTML) to your contribution as well, even if the comment is screened. These links will be forwarded to the mods of the community for confirmation.
