Well I didn't get as much done last night as I'd hoped, since I had the entire thread breaking problem (and there may have also been some breaks to cuddle with the cats, because it helps keep me slightly more sane. Kemuri and I had a very good trimming session as well, and I've nearly rid her belly of mats.) but here's what I did get done:

Here's what it looks like after I added gromits and ribbon for lacing, and also pictured is the spare too small top and the remnants of the fabric that will be used for the inserts:

So I cut the fabric into triangles and undid four of the side seams in the dress, yielding this:

After sorting out the problem with the needles, I managed to get one of the inserts in last night before hitting the point of being too tired to keep working. I am totally faking on this project, doing all sorts of stuff in my book of sewing cheating. I am, for instance, using seam tape on the inside of these inserts rather than trying to fold or serve or french the seams because I do not have that kind of time or skill and they would be bulgey and bad.
Still, not too shabby:

Add the other three inserts
Fix the ragged armholes on the vest top
runch up the skirt so it isn't going six inches past my feet
