For 3) I think you'll enjoy an older fic called 'Can't See in Your Windows' by sparkoffire. It has young hurt!Sam whilst they are being looked after by Bobby...it's a flash-forward/flashback format put to very good use:

While not really the main point of the story, Relative Gold by Dayspring covers request #3. Ten years after Sam is thought dead in a motel fire, the day Dean turns 18 John gives him the keys to the Impala, tells him the rental they're staying in is paid up 'til the end of the school year, and and disappears to continue hunting without the 'distraction' of having his son with him. When Bobby finds out he throws John off his property and finds Dean. He takes him in and enrolls him in his old college as his nephew. With full support of Bobby and Pastor Jim, Dean tries out for the US Olympic shooting team. (Note: it's not really a death story, just a lot of angst and guilt over the 'death' with a very happy ending.)
