Ok, so, follow my logic here.

That drug from tHoB worked by exploiting/amplifying fears, yes? And immediately after Sherlock was doped up he was all "I don't HAVE FRIENDS".

Sooooo by my logic one of Sherlock's biggest fears has always been that he would always be alone/never have any friends.

Bonus if he's had this fear since he was a little kid getting picked on for being different.
Double bonus if you add in a scene about how John saved him from his fears by actually being his friend.
