I think it would be great if season 8 opens with House still standing outside Cuddy’s and he was just having deranged thoughts of driving a car through her house and going off to a beach somewhere. Unfortunately it doesn’t explain the scenes at night, but I still don’t understand them anyway.
But the amount of drugs he was taking in the lead up could possibly explain some of his recklessness and violence (smashing up Wilson’s office) plus he was taking that experimental drug so who knows what else that did to him and what other side effects there are. And then he’s come to a stage where he’s thinking about hurting other people and quite possibly himself, regardless of that beach scene. Let’s ignore that. ;)

I really really would like to see this. And the scenes at night are supposed to represent after the crash, after House has already left to the tropical location. Apparently the time when he crashed was near sun down. But still...he could still be standing in front of the house, imagining the aftermath. And he was taking soooo many drugs.
The responses from the writers on Twitter seem to indicate that everything we saw was real. However, in an interview with the writers who co-wrote that episode:
So is that final scene where he’s at the beach, is that real or is that in his mind?
PB: That’s real. Well, I can say that right now. We’ve got a writers meeting next week, but let’s just assume the audience has no reason to think that it’s in his mind.
Ohh yeah I found it confusing because it seemed like broad daylight when the crash happened and then if it was night, it took a long time for emergency services to get there.

YES! This way it works. The whole thing (episode) stays a nightmare but at least House doesn't jump off the cliff and he is as shocked and afraid about it being possible to become what he in canon became as we are! Excellent. How I wish House would come to his senses and really get that if he really wants to destroy himself then Vicodin and booze and self operating and possible unprotected sex is not the best way to do it. And if he doesn't want to die, then he should do what he did here: give Wilson the pills, give Wilson control, and not just over his car. Good job.
Very nicely done. This way would have made so much more sense! Oh well. Thanks for sharing this version. Maybe I'll just pretend that's what happened for a while. :)
