This is excellent. I love the flashes of what's going on elsewhere, combined with Yoruichi's very pragmatic point of view, and the two doctors' analysis of the situation. The snow is the perfect touch. The whole thing feels so very ... desperate.

Wow, yes, the angsty desperate feel of this is awesome, and I love how vividly (more like bleakly XD) the scenery comes through. I'm also enjoying Ryuuken's emotional state, because i'm mean like that and I like to wonder how he's going to splinter at precisely the wrong time. Mwahaha. Good chapter!

The mental image of Kensei using one of the Urahara kids as an artillery stand (for lack of a better term) is hysterical.

Which reminds me: Things are going too well. Granted, we still see horrible things like Tatsuki dying, Ryuuken falling apart and Szayel creating living Hollows, but you also have to consider Ichigo coming back, Byakuya, Shunsui and Renji all turning up alive, Gin and Barragan going down, etc. In fact, if I didn't know any better, I would say the Soul Society side of the plan is a complete success.

Which makes me wonder how badly the Earth and Hueco Mundo side of things are about to go. Szayel won't be any fun, nor will Mayuri. Then there is the big question mark that is Aizen...I have the feeling things will get worse before they get better, but when is that not the case here?
