IA Santana's shown to be observant, she doesn't need much to come to a conclusion. With Blaine and Kurt's comments about Karofsky from NoN and Karofsky checking out Sam (even though it would be sublte I'm sure) she would easily put two and two together.

totally think it's karofsky that checks Sam out (in probably a very sublte way) that makes Santana put two and two together. She's shown before to be observant so it wouldn't take much when she already has Blaine and Kurt's comments rattling about in her head.
