I specifically remember Tia as a major troublemaker throughout this series (She totally tried to snatch Conner away from Liz) and since she *is* featured on the cover, I'd consider her a top contender for the title.
That said, its been a while (like a decade) since I've read the book but based on the plot of the particular book, I'd say Trent is the trouble maker, even though he's not on the cover. (But since when do Sweet Valley covers ever make sense?) He's Jeremy's best friend - he should know better than to get up in Jessica's grill. Not to mention his thing (though it might've been in it's off again stage) with Tia.
Tia and Trent were made for each other. Neither one has any respect for the rules -- as in you don't date your best friend's girl/guy -- past or present. You just don't.

Yeah, and she cheated on Angel with Trent and then went all waaaah! Sniff, Sniff, I'm so sorry Angel, I was so lonely and I just fell onto this dude's lips!
