Homunculus vol. 1 - 9, Yamamoto Hideo (365 Books, book 13 - 21)
Of course I had to stop right when it got really interesting (trans* characters! psychosexual development! modern society!) because I didn't have volume 10 and onwards. Homunculus is an excellent seinen manga with great art, but it should carry a trigger warning for rape (both a man physically raping a girl and the girl (... psychologically?) raping another person), as well as power issues, mental health issues, violence, surgery, and a number of other things.
Our protagonist is a successful young Japanese businessman who is taking time out to live in his car next to a public park. A medicine student pays him to perform trepanation surgery on him, after which he starts to see the manifestations of other people's psyches (The robot on the second cover and the sand girl on the fourth are such.)
