Midwives have been facing increasing difficulty in getting paid for treatment of Medicaid-eligible women since 1997, when New Mexico's Medicaid program was privatized and began to be operated by the state's managed care organizations.

Not only in New Mexico. When Medicaid was privatized, it screwed up many, many things. Hospitals that can't get paid, specialists that wont take appointments from patients with medicaid because they aren't guaranteed payment, etc. It takes the people that need health care more than anyone else, and makes it next to impossible for them to get decent medical care. It's not about keeping people healthy, it's about money.

Yeah, medicaid is nearly worthless, imo. In AZ, we don't have medicaid at all. The state has it's own system that is even more convoluted than the national system. AHCCCS won't pay for midwives at all, even though it's been demonstrated that it would save the government several million dollars per year if only 2% of AHCCCS-eligible women used a midwife.
