Patient has shown increasingly violent signs of mood swings. Past diagnosis of Bi Polar Type 2 seems to be on track although many failed medications have taken place over the course of the rehabilitation process. Lexapro, Depacote, Zyprexa, Ambien are the medications which have shown little to no effect in aiding in abating the signs of mood swings. Current medication is Lithium Carb ER 450MG. The levels taken on last blood collection show the medication is at its optimal theraputic levels but the symptoms have not gone away.
Patient has shown signs of a self infliction habit and anxiety coupled with a mild case of paranoia. The medication he tells has been doing fine by itself but he has also expressed the desire to seek a second opinion. At this point in time gathered from what I could with the details and observations I have made myself, I am unable to give a full diagnosis of the situation. The anxiety and mild paranoia could be stemming from extreme social, family and other related areas in the patient's life. He has expressed his desire to find other methods besides medication however it is in my opinion that to fully figure out and diagnosis what the patient suffers from we should use medications as tools in order to reach this goal.
At the current time the diagnosis is unknown, believed prior to be Bi Polar Type 2. These will be referred to the next following physician.
