See, I keep see-sawing on this for both of the reasons you point out. When I listen to the voiceover, especially in the very beginning of the first episode, it sounds like an older person looking back on a first love. But from the conversation with Ren and from that final image of Hiroto and Nao in white with closed eyes...these seem to indicate death. I'm leaning toward death for both, though, after the convo with Ren.
Re: Ren in wheelchair: I agree with you and ina - I wonder if there's more to it than just the asthma - maybe that's all that Ayuto knows? Maybe there's a problem with his heart, too? I don't know. I have asthma (although not as serious as Ren's) and I know others who do have serious asthma and it's not enough for a wheelchair, especially since pushing that wheelchair around will do you in if the asthma is that severe...Since his legs were completely inert in the bedroom scene, I think there must be more to it.
I think what you may have found forced in the scene between Hiroto and Ren might have been because of their awkward positioning. Because the room was so narrow, they had to face the camera in only one way which limited their physical interaction. As much as I loved the scene, I wondered at the (un)naturalness of the pose. I suppose the director would explain it away by reminding us of Ren's unmoving legs...
The one thing that DID stand out to me is something I've noticed with Takuya Kimura as well...often when he's trying to be comforting or soothing, the hand doesn't linger in a caress - rather, there is a series of small pats, and in Kimura's case they can look harder than is naturally warranted. I have wondered if this is a Japanese thing or something typical of him - but Kame was doing it as well. I would much prefer the lingering caress which has more warmth than staccato pats (yes, I realize I'm being uber-nitpicky).
Koki...hmm. He's fun, but too exaggerated. Especially for my sensitive western sensibilities! ;) I agree - Koki, lay on some depth, and I'll love you forever! :)
