Dear Jeremy,

Per your request, this letter is the result of your psychological evaluation and treatment from May 19, 2004 to June 17, 2004.

The results are based upon your psycho/social history that you reported to me during therapy. You reported that you started using marijuana in high school to alleviate your symptoms of depression and progressively became dependent on the substance to cope with low self-esteem. You reported marijuana initially helped you become more insightful, communicative, and extroverted which improved your self-esteem. However, you reported long-term marijuana use made you feel even more depressed, anxious and introverted. You reported that these later symptoms were the former symptoms that you experienced before you started using marijuana. You further reported that your depression and marijuana use not only affected your self-esteem and social life, but your college grades as well. (Academic Probation)

My psychological assessment of you suggests that you were experiencing symptoms of clinical depression in high school and used marijuana to alleviate your mood swings to feel normal. A combination of the initial onset of your depression in high school and long-term marijuana use only further exacerbated your depression. As a result your current diagnoses are Major Depressive Disorder, Recurrent, 296.3 and Cannabis Dependence 304.3. My previous recommendations to you were to obtain a psychiatric evaluation for anti-depressant medication and intensive outpatient therapy (IOP) for substance dependency at Maple Grove Hospital in West Bloomfield, Michigan. I further recommended that you continue individual psychotherapy for your low self-esteem after you are stable and sober in your recovery program. Your prognosis is good given you follow that above recommendations. Therefore, your self-esteem, social life, and college performance should improve with your abstinence from marijuana and therapy for depression and self-esteem issues.
