You're seriously asking people on a Caravan for a prosthetic hand? First of all, that's ridiculously stupid, second, it's like you're just asking for gangrene. All that fancy doctor mumbo-jumbo about sterile environments is, oddly, not actually mumbo-jumbo.

[House understands, he really does. He was about a step away from losing his leg, but made a stupid decision and has to deal with it. Maybe, in some convoluted way, it's his way of caring.

Mostly.

[ that gives him pause; what other kind would there be? oh well. no harm in explaining ] My idea? Well, where I'm from, the loss of limbs is commonplace. So, prosthetics need to be top-notch. They're all fully-functional replacements, full range of movement and stuff like that.

I didn't realize it was so commonplace. What year is it where you come from?

--Ah. [ okay he chuckles ] No, it's not quite like that. Most of it is work-related accidents. Even with advanced technology to protect miners and engineers, there's always issues.

2009. Not exactly at the fully functioning replacements level yet. [Really, he has no reason to be bitter since he doesn't have a prosthetic, but he still wishes that he had chosen it sometimes. Rarely.]

What's your deal, then?

Obviously not for everyone. I'm have the wonders of the PSP, cable television and my 1967 Gibson Flying V, and that's all I need in life.

Well, and hookers, but hopefully that's common to any time.

Sucks for you. That just sounds like a messed up version of Space Odyssey, so looks like there's nothing I could have done anyways. Have fun trying to not get gangrene, though.
