User-info, Subject # 00183: Subject awoke from sleep and immediately cuts great swaths out of his hair. The Staff wonders what brought this about. Staff suggest change in subject's medication dosage, as he appears to be sluggish and uncreative. Subject is also displaying characteristics of impending suicide, he no longer eats much and 2 days ago was caught making a noose out of dental floss. When asked about this he replied "I am waiting to die." Staff request that a psychiatrist be added to roster of physicians currently observing subject 00183, this might lend understanding into subject thinking operandi. Staff out.
