Don't know what has and hasn't been done, so ignore me if this has been covered.
Have you talked to the vet about steroids for him? They're generally a good quality of life measure for pets that are on their way out. They help suppress some of the pain, especially any that is inflammation related, and stimulate appetite. The normal down side is that they can cause long term liver issues, but that doesn't matter when you're just trying to make their end as best as possible.
We have an elderly ferret that has had diabetes for a couple years and cancer for a year or so. He started succumbing to them hard about two weeks ago and is currently on a ridiculously high dosage of steroids every day. With them he's at about half his old activity level, but is able to still move around and play with the others when he is active. Prior to adding the high dosage, he was looking like he needed to be put down in the next day or two. He's definitely still fading and will likely be gone within the next month, but it's been worth it.
Also it's about 12$ for a month supply at ferret dose levels. Cat will likely be higher, but it's still a pretty cheap option if the vet thinks it's worth it.
