Right now, only Income and Aviva have plans that reimburse on a as-charged basis, ie no sub-limits, for Shield-type plans. Most agree "no sub-limits" is the way to go. However, Income requires patient to call in few days before admission to get approval for the treatment. That is how they "pay attention to prevent escalation of claims". Both companies also offer riders which can cover the co-insurance and deductible portion.
