Birthday/contact details role call.

Having lost the notebook I had peoples birthdays scribbled in I am giving you the chance to remind me so they can go in next years diary which I am starting to add stuff to, so:
If you give me your day and month of birth i will wish you happy birthday in some form.
If you also give me name and address as well I shall endeavour to try not to forget to send you a birthday card as well. I might even send you a christmas card!
If you feel like it chuck in other stuff such as phone number, email etc.
And if you are really bored tell me something amusing too!
