Here's the email I got back from the publishers:
Dear Nicole,
I'm sorry that the copy of "The Only Life That Mattered" that you purchased was defective!
As you probably know, most books are assembled in units or "signatures" of 8, 16, or 32 pages, which are then bound together. Occasionally, at the printer, because of either human or mechanical error, the signatures are assembled in the wrong order, signatures is missing or duplicated, or a signature is
blank.
Our printer hand inspects every fifth book exiting their bindery. Any
problems that would affect large numbers of books are caught, but,
unfortunately an defective book occasionally slips through. We are
very sorry that you received such a defective book.
