Ok, I started with this picture:


and cropped it to a 300x200.


Duplicate the base and set it to Screen, opacity at 100.
Make a new layer and add the color #ebf4a4 and set it to Soft Light, 100:


It's ugly right now. :P Now, go to Layer: Colors: Color Balance and do the following settings:
--Midtones--
-48, -21, 80

Duplicate the yellow layer and bring it to the top, leave it at Soft Light but put the opacity down to 29.8.
Make a new layer and add the color #f1a4f4, setting it to Soft Light, opacity at 29.8.
Do the Color Balance with the following settings:
--Shadows--
80, 0, 0

Next, go the Layer: Colors: Hue-Saturation and set the Saturation to 30. Merge your layers.
It should now look like this:


I made a new layer and added this texture by _iconographer and set it to Screen, 100:
sorry for some reason I didn't save the texture! ):

Cut out of the texture so you can see Dom's face and shirt. Duplicate that layer twice and set the duplicates to Burn, opacity 100.


Make two new layers and add the color #6db9f3 to both at Burn, 100. For the second layer, cut everything out except Dom's shirt.
That's just something I thought made it look nicer. You can get rid of the second blue layer if you don't like it. :P
Now, make another layer and put the color #ffd7cb setting it to Multiply, 100:


You can stop there if you like. :)
I went on and duplicated one of the texture and layers and brought it to the top setting it to Multiply, 40.
I made another layer and put the color #041e54, setting it to Screen, 100. Duplicate that layer twice and set the top to Difference.
Duplicate the base and bring to the top setting it to Soft Light, 49.8:


Duplicate the texture layer and bring it to the top, set it to Soft Light, 100.
Duplicate your base again and bring it to the top. Desaturate it and set it to Burn, 100.
Now I added a brush by (?latex?):


That's it! After that I used the font Hotel Coral Essex for the "Happy Bday" words. :) Hope that was helpful and I'd love to see your results!
