Well, it would mostly depend on the human, and how it's explained to them.
Some are more open minded than others, and therefor would likely have an easier time understanding it.
At the same time, in cases of John for instance, they may still have a harder time grasping it no matter how open minded they are, and may need it explained in a more human-relevant manner before they'll fully grasp it.
I'll admit, I honestly don't believe I truly fully grasped the concept until I sat down and spoke about...Karkat and our relationship with John after he left...
Which...really pains me to say, but...
It generally isn't that hard of a concept to grasp. Much easier than if you were to try and explain any form of blackrom.
Especially as it mirrors various human relationship and feelings so well.
It's merely a task of connecting those human based emotions with the concept of a single, committed relationship is all.

Does he at least show any interest when various troll subjects are brought up, despite how little he doesn't seem to understand them?
You could always breach the topic of romance in general, explaining that to him at first, to judge how well he grasps moirallegiance.
I found troll romance very interesting, and because of that Karkat was able to explain it to me long before the idea of becoming moirails ever planted itself within his head.
That, I'm assuming, made asking me an easier time for him, as I proved I at least was able to understand what it entailed.
I would be plenty willing to help you explain it, should he require it.
