Size Matters is amazing, and also a lot of fun to work with, because it creates a situation in which Elena's and Damon's POVs MUST be very, very different :D. Obviously Damon isn't obsessed with his own penis ;) (Though, who knows...)

I've seen people say Damon worries Elena wants him because of how he looks, and others say he's much more confident than that. What do you think?

I think it actually depends on a day. Elena herself doesn't always react to him the same way. Sometimes she clearly cares about him as a person (see: Denver), and sometimes she's shamelessly focused on the physical (see: "Are you staying for the show?"). I think Damon picks up on it, and that's why his attitude is often ambiguous. He knows she cares about him as a person, that she genuinely wants him for who he is. But he also knows that there are days when she'd be perfectly capable of just treating him like a sex toy. I'd say he's confident, but he's also aware that Elena isn't fully stable.
