That line is a great explanation of how Elliot has managed his life. He's tried so hard to separate himself from everything that he no longer has anything to hold onto.
He saw his own futility. His own powerlessness. He saw his future, his past. An endless series of kids he was too late to help. Or not there to protect.
Yep, Elliot for all his attempts at separation tends to get caught up on what he couldn't do.
I really like the bit about him feeling his heart beating -- it's a nice soothing moment after all the angsty thoughts.
Nice look into what was going through Elliot's mind after they so abruptly tossed him out of last night's episode. One question, who is the "she" he goes to? I'm assuming it's someone from an episode I missed (I often have a brain freeze and forget SVU is on). Or is it supposed to be vague?

It's supposed to be Alex, the ex-ADA who's living somewhere in Witness Protection. The name of the piece, "Altoona," comes from a running joke we have on the TWoP SVU forum - that Olivia and Elliot take turns visiting Alex in Altoona and getting, er, comfort.
But disregarding the title, it's vague enough to be just someone Elliot's found. It could be the women from "Debt," earlier this year - not sure if you saw that episode, about the Chinese woman searching for her niece. Or it could be someone never seen on the show.
