It's been a while since I've read this one and I don't remember the title. It was a COE fix it, where Ianto was somehow resurrected on an alien world to serve as a conduit for the portal between that world and another. Somehow the Doctor manages to find out and he, John Hart and Jack mount a rescue, though I don't think John arrives with them as I remember a scene where he comes through the portal and he and Ianto see each other.
