Here's an idea I hope someone can use. This is a darker view point then most ideas I come up with.

Ratchet's home village is attacked in the night and set ablaze those that weren't killed were captured. Ratchet is singled out with other carriers to be sold. Carriers are rare and hard to come by, some will pay anything to get one.
Ratchet is bought by a mech named Tracks for a pair of Twin Lords. They need heirs and have been unable to find a suitable carrier. At first Ratchet is nothing more then a way to produce sparklings, the Twins using him however they see fit. Often leaving him exhausted or injuryed.
As time passes the Twins discover how much they've come to care for the gruff yet gentle Medic but the damage as already been done. Ratchet doesn't trust anything they say or do and wants nothing more then to excape. Past tries have not ended well however, he'll stop at nothing to be free again to protect the newspark he carries.
Can the Twins fix what they've done or will Ratchet excape and never be seen again?
