I didn't see distinct paranoia as an element listed in the description of NPD to which you linked. Still, if you think everyone around you is jealous of you, that would be paranoid if not diagnostically paranoiac.

Self-diagnoses being as trustworthy as they are, this is illustrated by the link Elf provided, which would also highlight the problem of finding research subjects. A narcissistic person wouldn't admit that anything is wrong, and how can you tell that someone has a problem if they don't complain of it?
From my experience with people with NPD and PPD (paranoid personality disorder) as well as paranoid schizophrenia, I haven't seen or heard of a clear case of paranoia mixed with narcissism. The distinction I see based on my own studies and experiences is that someone who's paranoid is 1.) worried about things that are completely impossible and 2.) feels that their beliefs (paranoias) are that someone/something is out to cause harm. Narcissistic people may believe things that are completely impossible (like this song is completely about ME), but they see it as a positive thing. "This song is about me because I'm the greatest thing on this planet!" rather than, "This song is about me because the people who are trying to kill me are using it to convey a threat." What about the assumption that any last thing which the person chooses to take offense at must obviously have been done specifically and expressly to inconvenience/get back at/target that person and only that person, not necessarily because "they're so wonderful" but because everything is always all about them? That still qualifies, yes?
