Outside the box can be seen a few ways, at least in my opinion. It could be a icon where the subject in the icon sort of 'pops out'. Or literally coming out of a box (like a border). Examples:

Or a tv character doing something that is completely out of their normal mannerisms (example - a gloomy character being happy).

Or really unique coloring/cropping/texture work.
