GUIDELINES:

You can submit the form up to three times, but for every request you submit, you promise a fill.
Any and all kinds of fic/media will be accepted for fills. You can even do more than one for the fill. It is the holidays after all. These are the minimums:
Fic: at least 1000 words
Icons: a set of at least 5
Fanart: a completed coloured drawing
Graphics/Banners: 2 banners/manips
Fanmix: 1 mix, at least 6 songs, coverart if possible
Fanvid: 1 video, 1 minute minimum
Note: ALL Comments will be monitored to keep all identities a secret.
