pull the disk out. Attach to a USB/IDE converter, plug in to a known clean system (failing this, run msconfig -> selective start up -> untick everything do it in SAFE safe mode ;P)
Run HiJackthis, Spy-bot Search and destory (patched), Ad-aware and Windows Defender (it will scan registries, even when they aren't live).
Finally hit it with a decent virus scanner (NOD32 or Kasperspy) and then reinstall the drive into the machine.
Hit it with a defrag (or 2 if you are using MS's one), clean up unused services, temp files and then hand it back to the client.
I haven't found a "malwared" or "virused" pc i haven't been able to clean up yet.
Sounds like you have a dying hard drive (check your SMART Status, use your vendor's HDD scan tool) or RAM/PSU problems.
I hate to say it but i don't think your problems are all gone yet (altho going through a windows install is a reasonable test).
Finally - System Restore is useless, i've never seen it work when you'd think "Hey, System restore will get me out of shit here". I think its more directed at removing software than repairing windows.
This thing is definitely dying. It's over 7 years old and every 6 months or so for the past 2 years will have a melt down. I just need it to get me through to July/Aug so I can get my tax cheque and go get a new one.
