So now that everyone has signed up, I need you all to fill out this form so I have information about everyone. This information will be given only to your Secret Santa. All posts will be screened so that only I can view it.

Name is for the name that the package will be addressed to. The question regarding gag gifts is there for anyone who only wants serious gifts and nothing funny or silly. And the allergies question is so that your Secret Santa knows if there is something specific s/he should not get you.
