What is the biggest lie you ever told? What were the consequences?

When the Grand Admiral died, I knew that I had to impersonate my template or be used as cannon fodder.  I rewrote the records.  I reprogrammed a medical droid to have absolute loyalty to me, and told it that I would be killed if it didn't help me.

On my orders, it bored a hole into my abdomen that could pass as the scar left from the umbilical cord.  It burned away my fingerprints, and inflicted minor wounds which were then incompletely treated, to mimic his other scars.  There weren't many.  I spent two weeks in sickbay, taking the medications and treatments that would tweak my biochemistry until it read exactly the same as his.  The medical droid performed a perfect trepanation, modified so that any difference between my personality and my template's, any fudged recollections, could be explained as the result of head trauma.  Soon after this the Reborn Emperor surfaced, and in the confusion my actions went unnoticed.

When it was done, I told the galaxy that I was my template, that the word that he had died had come prematurely.  To his superiors, to the men and women who knew him.  To his family, and his fiancee.

They believed me.
