I have a feeling that a change in surroundings will definitely affect your out look on things as they are. I think a large portion of the rut you are currently in has to do with the fact that you are back in the same general place where these problems arose in the first place. Living in another place where you can get a fresh perspective will more than likely give you the will power, and ability to see through some of the problems you are having trouble resolving.

I know how it feels to be stuck in a rut, and in all honesty when I came back to Arizona I fell down pretty hard, because I had worked so hard to get out of this place and yet I am now back. Luckily for me I had some influence to help me along the way, and put me back on my feet.

For you I think aside from moving to a place with new surroundings, and plenty of friends, finding a person, or persons, that can help bring you up and keep you motivated would be a great help. I know that where you are now you don't have many friends that are near by that can help motivate you towards bettering yourself in the ways that you desire.
