The entire scene from the point where Sherlock appears outside the surgery to their parting - you have their voices spot-on. John throwing Sherlock's phone into traffic is perfect - Sherlock *would* be fool enough to keep texting without paying attention to John even at that point, and it *would* be too much.
Even if Sherlock had told John that he'd broken matters off with Lauren at that point - which I gather he had, from "You Are an Emergency" - I don't think John would have believed him. Why would he? As John observes here, Sherlock is too good at faking emotions - and, in the ambulance scene before Sherlock had what I suppose was his final meeting with Lauren, Sherlock had already said enough to *show* John that he'd been lying to and cheating on John up through that afternoon.
He never gets around to changing his mobile number, just in case.
Perfectly believable. You don't just turn all that (makes unseen hand gesture in the air) off just like a snap of the fingers, because the breakup is official.
Back to an earlier point in the story - the marks on Sherlock's skin that provided the final piece of evidence. I gather from putting this story together with "You Are an Emergency" that they actually weren't marks left on the afternoon of the bicycle incident, but were left some few days before, possibly. I take it Sherlock must have been incredibly careful not to let John see him in good light with his shirt off prior to the bicycle incident. It really *was* just a matter of time until the whole thing came apart.
Even if Sherlock had kept everything hidden until Lauren's fellowship was over and she went away, assuming that really *was* how things would have played out - I daresay some pictures might have turned up. Or he'd have kept messing about with her now and then until he finally *did* get caught.
