You can leave out the personal details, if you would prefer, but I do need to know whether or not the Hulk comes from gamma radiation.

I see. The Hulk that I knew was the product of Bruce being caught in the blast of a Gamma Bomb, being completely saturated in highly radioactive particles. It might have been wishful thinking, but for a time, Bruce had lost his ability to transform into the Hulk once the radiation had been removed.

Some are in use, or I assume that they are used when I am in Hulk form.

Some of them, I don't know if I want to know what would kick them into activity.
