This is gorgeous and insane and also a bit heartbreaking, and the words you use and everything and guh. You've effectively turned my brain to cat litter.
I kept thinking Inception every time you mentioned a dream within a dream, by the way. I don't know if that was intentional, but it fit the mood rather well.

She stares at him, and she can see all of him-thin, translucent images layer themselves over his face. His faces. So many of them. She laughs, because she can see his hair, white and black and gray and brown and gold, all at once, everything at once. "Like doors," she says, nonsensically.

I actually watched Inception for the first time last night, so of course that popped straight into my head. But this is a really different way of looking at divergence: did the Doctor leave Rose at Bad Wolf Bay? Did they never come there? Or did she never leave?
I love how you got that in there, right in the middle.
