Notes: Okay, this is the closest I can come to explaining this.

Future!Peter kills Nathan in the future (304), then teleports into the past to kill the Nathan there (211) to trigger events that will cause Sylar and himself to be together. After he's killed Nathan (301) he then teleports into the future where the timelines have adjusted and he is with Sylar (304).

This is obviously not chronological in the vid, though.

So...

Basically the Peter/Nathan scenes in 304 are of future!Peter and Nathan, while the 301 scenes in the hospital are present!Peter mourning Nathan. And all of the Peter/Sylar scenes (except for the ones at the end) are various moments after Nathan has already died.
