He's staring down at the arc reactor in his chest and holding the camera out with one hand so the top half of his body is visible. The spiderweb vein patterns from the beginnings of palladium poisoning are also apparent, branching out from the arc reactor, extending about an inch and a half in a sort of border around it.
There's a whole slew of empty mini bottles on the desk he's sitting at which can be seen as the camera shakes when he talks.]
