Yeah, they totally believe in intervention. The nearest hospital is only a few minutes away and they won't hesitate to call an ambulance if it comes to that.
HMO's almost universally refuse to pay for midwives, but we have a PPO. We'll probably have to pay "out of network" copays but we should be able to get them to pay for at least part of it because the facility that we've chosen uses CNM's which is just another form of registered nurse. If we wanted to use a CPM we'd likely be paying out of pocket. But... the cost of a midwife attended birth and prenatal care is usually about 3500.00 and some even have a sliding scale. I don't think i need to tell you how that compares to a hospital birth. I think the bills for Aaralyns birth that the insurance covered were in excess of 25k. It's cheap enough that i won't be heart broken if i have to pay the whole amount out of pocket. I've got a credit card i can put it on if it comes to that.

The author of that sounds like she's got a good idea of what a midwife attended birth should be.
As long as your midwife is open to medical intervention IF NECESSARY, then a home birth is a good option, provided you are low risk.
Does your insurance cover midwives? Many won't.
