First fort: entire population decimated by a 'forgotten beast' which wandered in through the underground.
This beast was a quadraped composed entirely of vomit, had two tails, and breath which literally rotted dwarves faces off. I shit you not.
After being almost ripped apart by the dwarven military, he went on a rampage rotting everyone else in the fort, who were unfortunately unarmed and unable to kill him.
