i was there on business, so the means to get in was quite different. but even as a tourist, it is not difficult, prob a little more expensive than your typical destination.
you will need to get a travel agent that you could work with for your tour - select how long you want to be there and where you want to visit. based on your intended itinerary, and upon payment online, they will apply the visa for you. Note that the visa includes your tour and the typical charge would be USD200 per day for low season and USD250 day for high season. this fee includes accom, food, tour guide, transportation etc - the works, and you might be put up in from 4 star hotels to cheap motels depending on the town you visit. 35% of your fee goes to the govt for the country's healthcare and education system, so you are also helping the people when you visit them !
with the visa approved, then you are permitted to book your flight there. as druk air (the only airline flying to bhutan) goes to only 3 - 4 destinations, you will have to transit either at dehli, bangkok, nepal (check the druk air website). so thats that in summary. feel free to drop me a line if you need more details.

1. yes you can go alone. well at least i did, but i was also a guest of the govt.
2. the agent was recommended to me by some of the govt officials i was having meetings with. drop me a note at tarzansg@yahoo.com and i'll look up the details for you.
3. best time to travel - now till early dec (which is high season), but you have all the hordes of tourists all going there to watch the festivities. else go in spring. i was told by my guide that dec is also good if you are ok with the cold, where you get the clear blue skies and low season rates...
