So, I'm guessing these are grad school applications? If so, congrats on giving it another try :-) Now to answer some of your questions:

1. Yes, you do have to list ASU, as well as OSU (both undergrad and grad).

2. No you do not list Carleton and Trinity in this section. Those were summer programs--you weren't actually matriculated at those universities. This section is so that they know where to expect transcripts from, and they do want them from all the schools, whether you like it or not ( ... )

See, the problem is that I still owe them money. I don't know how, since I originally understood that "tuition wouldn't be a problem", but I owe them money. So they won't send my transcripts until I hack up a very large amount of money.

eeep. That certainly puts a wrinkle in things. Have you talked with the head of the math department about this misunderstanding? He'd be a lot more patient and informed about clearing things up than the bureaucrats who deal with billing for the school. Perhaps somebody forgot to fill out some magical paperwork and you actually shouldn't have a balance?

No, as it turns out, its the balance I should have, although I would have liked to know that I would be expected to pay tuition BEFORE I went to school there. There's nothing I can do about the tuition thing. The barrier lies in the sad fact that they won't send my transcripts.

Crap. That really sucks. I see your dilemma now: if you don't tell them you were at ASU, then they will undoubtedly ask what you were doing for that year. As soon as they find out you went to ASU, they want transcripts. And as soon as you tell them why you can't send them transcripts from ASU, they become highly supsicious of your financial reliability and that can be a fatal blow to your application. Crap ( ... )
