I said I wasn't gonna do this - voice my opinion - since I so rarely seems to agree with fandom theories, but apparently half of it is already in this post LOL.

Cas said that he couldn't fix it. Ergo, there is no saving of Sam by anyone, even angels. Hence, this means Sam is still crazy.
Cas said that all that he could do is redirect it. What he did was not replace, remove, or otherwise stop Sam's hallucinations and madness. What Cas actually did was create a new connection - a psychic link between Sam and himself - and he is actively porting the hallucinations over from Sam's head to his own. He doesn't need to eat (and therefore doesn't need to sleep) and so he is already massively more qualified for dealing with the debilitating side of this situation for as long as Sam needs him. Talk about having angels watching over you!
Erstwhile, Sam is able to function, mostly. He will be able to sleep, think straight, eat without fumbling over candy wrappers, and otherwise be Dean's brother... as long as Cas is safe and secure in that ward. This was the only thing that Dean was, rightly, concerned about. (Props due to Meg for ensuring that by posing as a nurse remains to be seen.)
The hallucinations and madness, once ported over, will morph into the things which Cas fears most. That is to be expected. Once the voice reaches his mind, it will naturally change to be about the things that Cas himself would find terrifying or debilitating, like an effed-up game of (BANANA) telephone.

I feel like the least over-thinky explination would be that Sam was crazy; Cas couldn't make the crazy disappear, and so he shifted it("this isn't a problem I can make disappear. You know that. But I may be able to shift it."), out of Sam and into him. Like if Sam had a cold and Cas was all, "I'm sorry, but there's no cure for the common cold (obvs!). But I can have the cold FOR YOU." And then Sam wouldn't have a cold anymore, and Cas would.
