Uhm, what effects?
First I made the pic b/w and added a dot
just with a round soft brush, then I draw a white heart
with a small round brush and added borders...
I hope you know what I mean =P,
if you don't, I will try to make a tutorial :)

i think I have a pretty good idea. I just thought it was an effect with the lighting and maybe some sort of stamp you used for the hearts.
