So its around 4am and we get a code blue in ICU... I have been expecting the call for some time, well since they took the crash cart in there to be on the safe side lol.
Pt has ascites in the worst kind of way, his belly is like a drum...and his peak pressures have gone from an ok 22 at 8pm, to a oy vey 54 at 2pm... not surprising given that big ole belly pressing up against his diaphragm. Doctors are made aware early on, but nothing is changed...the nurse is maxing out her pressors trying to get his BP out the toilet... its all going pear shaped... the guy is only 52 < sigh >
So 20 minutes into this code... no rate, no rhythm...his head and feet are blue. The residents barrel in... (poor things really need supervision) One looks like they are a deer in the headlights, the other is very very confident... even though we are doing compressions, pushing every drug on the ACLS protocol, she says...
"Do his lungs sound wet? Lets give him 40...no 80 of Lasix."
