Shepard only lives if you choose Destroy and have a high enough EMS. I just got done with loading my Insanity playthrough and re-ran the final push (love the "three husketeers" name you gave it, btw), and I interrogated that starbrat like he had relatives in ze Old Country. And he explained that with either Control or Synthesis, your body gets destroyed.
Anderson will die regardless, but the way you can get the option to talk TIM into shooting himself is to take the Paragon or Renegade choice in every conversation with him before that - some of which are not immediately obvious when you talk to him. Basically, if you're talking to him and don't see a blue or red choice, go with whatever dialogue option is available on the left side of the wheel to make it available.
The saddest thing about the EC is that, if this had just been in the damn game to start with, people would have hated the endings a lot less.
