I'm not sure what the right answer was supposed to be. I don't know if she's planning to move into the clinic permanently and be available around the clock or just sneak in and out whenever she decides it's necessary.

Honestly I think the best thing would be to have some dedicated security guards at the clinic. Either visible or working in non-medical positions like the receptionist or the janitor. There are some combat-trained medical personnel like myself, but it's very hard to keep an eye on security when you're concentrating on a patient. Most of the staff change over monthly with the regular mission postings too. I intend to have mandatory security drills several times a month, but I can't expect to have a complete staff of battle-trained medics.
It would be much easier to have her work alone and be called upon as you see fit, but I cannot decide for her.
Having guards would be beneficial, but I would not have them be visible. If the Purists see merely in passing the guards, they will put more strength into the attacks. It would be better to have our enemies be caught off-guard and unaware of the guards. I will offer my assistance should you accept it, but I cannot guarantee that I can be there often. Should you decide to have guards, I can offer to train them myself.
I'll defer to the experts on the details. I worked in field hospitals when I was in the army so I'm aware of basic security precautions, but that's obviously not my area of expertise.
The problem may be finding the guards. Unless they manage to sign up for one of the positions, we can't really afford to pay them much. And the Guide doesn't always cooperate with job assignments.
I do not trust the Guides. They are unreliable at best. Perhaps Lady Une may be able suggest men and women from those in Preventer. I work for her as well so I may be able to inspect those who are willing to assist you. There may yet be a way for you and her to arrange something of benefit for the both of you, but I do not know if that is possible. There are other means of payment aside from coin.
