Some NASA folks were asked to investigate the culture at NASA and come up with ways to improve innovation and open-mindedness. One of them, an Astronaut, put together this video to highlight the problems with the bureaucratic nature of NASA. I think what's demonstrated here can be seen in any large process heavy organization, however.
