Why do some people attack civilians for no reason? Not even because they hold the city hostage or anything, just because. It's so... cartoonishly evil.

Cartoonish Evil is sometimes the only way someone can express themselves. When you're evil in a world, you don't get a reputation by being subtle and underwhelming, you lash out and make things explode. At least that's how this world seems to work.

Those people sick in the head. There's all sorts of sociopaths out there and a good amount of those who we hear about are horrible people.
