Sam was never afraid of Dean. Not once, in all his life and everything that happened to him, was he ever afraid of his big brother. And when he found out about what Alistair put him through in Hell, that didn't change. Not one bit. When he found out that Dean enjoyed it, it still didn't change a thing. They all had their issues, and some were a little bit darker than others. It was fair. Sam wasn't afraid.
But when he made the decision to take a swan dive into Lucifer's cell, all sorts of new fears popped up out of nowhere. Fears that back home would've held no weight because he couldn't afford them a second thought, but here, in the City, where all those thoughts and worries had time to fester and grow?
Well, it's no wonder that Sam's nightmare ends up with him on Dean's rack. Strapped to a table, and his head's being restrained, but there's nothing to stop the screaming. The pleading. Dean's meticulous, surrounded by an array of knives and needles and things the imagination can't even keep up with. And the blood, so much blood, but then there's really no blood at all. Each slice, stab, puncture, tear, draws cries that threaten to rip Sam's vocal chords, but then there's nothing to show that it even happened at all. All the pain he could imagine, and then it only starts all over again.
Screaming, crying, begging Dean to stop but then he's laughing. Laughing, and isn't it a twisted, hollow sound? Not quite like Lucifer at all, but the restraints are gone along with Dean's tools of persuasion, and there's the archangel himself, standing up in a stark white suit against all the darkness. Don't blink. There's a flash of lightning and the shadow of great wings cast against the wall behind him, and then he's smiling.
"I suppose I should thank you, Dean. But if you don't mind, I'd like to find my daughter now."
