Sam and Dean travel to Fayetteville, Kentucky, to investigate a series of snake bite deaths in a town that has a snake-handling church, though the deaths did not happen at the church and not just to church members. One death was a church deacon, the other a local social worker.
At the church the boys meet Reverend Winston, who tells them that unless they're there to join in worship, he has nothing ot say to them. They go to the local library to do some research, where they meet young librarian
Sally. She and Sam seem to hit it off right away. She gets them started on their research.
Sam reads about the death of a little girl at a snake-handling service about 20 years earliesr. The Sheriff shut down a lot of serpent-handling churches after that, and the only one still open is Reverend Winston's - the same church where the little girl died. It seems her parents didn't give the practice up - the father, Micah Avery, died of a snakebite eight years later. Sam and Dean speculate that maybe one of the snakebite victims is haunting the town.
The local family court judge is killed that night by a snake. Sam remembers that the family whose little girl died had a younger child and that protective services took her away from her parents altogether. The Averys took the case to court and lsot. The boys surmise that Micah Avery is the vengeful spirit. They dig up his grave and salt and burn his bones, then decide to keep an eye on Revernd Winston. He's the most likely next victim, so if nothing happens to him, the case is closed.
That evening, an asp tries to attack Reverend Winston, but Sam and Dean stop it. It was a real, not a ghost snake, leading the boys to think they are dealing with witchcraft or black magic. They wonder if Liz Avery, mother of the little girl who died, or the surviving child are the culprit. Dean decides to go to to Reverend Winston, while Sam heads to the library to see if Sally can help him find out what happened to the surviving Averys.
Sam catches Sally just as she's closing up the library. She agrees to stay and help him out. While discussing local witchcraft, she mentions that her grandmother was quite the potion-maker.
Dean learns from Reverend Winston that Liz Avery is still part of the church, and that Sally is the surviving Avery child. Back at the library, Sam sees a sign on Sally's desk identifying her as Sally Avery. He accuses her of witchcraft. She denies it. While they are fighting, an asp attacks Sam. Dean arrives and chops the asp's head off before it can bite Sam. Once everyone calms down, Sam and Dean decide that Sally is probably not the most likely suspect. Just as Sally realizes who the witch realy is, Dean realizes that the snake he killed is not the snake Sam shot - there's another snake.
Sally takes the Winchesters to her mother's house. Liz Avery is inside practicing witchcraft with her asp. Reverend Winston is there as well, admonishing Liz and ordering her to stop. Liz orders the snake to attack Reverend Winston, but he catches it before it can bite him. Sam and Dean destroy the Black Altar and Reverend Winston lets the asp go. It bites Liz Avery before Sam kills it.
At the hospital, Sally tells Sam that her mother is going to live, and that she's going to prison for first-degree murder by snakebite. AS the boys prepare to leave, Dean tells Sam one more thing he found out - Micah Avery didn't die of a snakebite at church, he died of a snakebite at home. He and Liz had just re-filed for custody of Sally. The boys speculate that Grandma used witchcraft to kill Micah. Reverend Winston sees the boys and tells them not to come back around. And also tells Sam that he's been marked.
